Prognostic Value of Peritoneal Lavage Cytology in Gastric Cancer

Özet
The microscopic detection of free peritoneal tumour cells in peritoneal lavage fluid in gastric cancer patients is a useful predictor of peritoneal recurrence and poor prognosis. The aim of this study was to verify the prognostic significance of intraoperative peritoneal lavage cytology, and the correlation between tumour marker levels, peritoneal lavage cytology and overall survival. We evaluated the presence of free peritoneal tumour cells with cytological examination in a series of 55 peritoneal washing samples from patients undergoing operation for gastric cancer over the period from 1997 to 2001. Twenty-four patients (43%) had positive lavage cytology. All patients with positive cytology presented serosal infiltration (T3/T4). Positive peritoneal cytology was a predictor of poor prognosis and peritoneal recurrence: the 12th month survival rate and the median survival time were 8% and 5.5 months for positive and 58% and 19 months for negative cases; respectively. No correlation was found between peritoneal cytology and gender, age and CEA; however a correlation existed between CEA and CA 19.9 positivity, and between CA 19.9 positivity, stage and peritoneal cytology. Intraoperative cytological examination of peritoneal washings can detect the presence of free malignant cells in the peritoneal cavity and can be used to select patients who may benefit from intraperitoneal chemotherapy.
