Retinal macroaneurysm (RMA), first described by Robertson, is an acquired retinal pathology that occurs as saccular or fusiform focal dilatations in the large arterioles of the retina, usually within the first three rows of the bifurcation at the branching points of the retinal arteries. The focal dilatations varied from 100 µm to 250 µm in diameter. The superior temporal arteriole is the most commonly affected. Patients are mostly over 60 years of age and are more common in women (60-80%). RMA is usually associated with systemic hypertension and atherosclerotic cardiovascular disease. According to the Beijing Eye Study, the reported incidence of retinal arterial macroaneurysms in the adult Chinese population is 1 in 9000 eyes. RMA is more common in the geriatric population and is associated with systemic diseases, such as chronic hypertension, which have a high incidence in the geriatric population. The patients are usually asymptomatic. Patients may present with visual loss due to macular edema, retinal hemorrhage, or aneurysm rupture. RMA usually involves one eye (90%). Considering that it is unilateral, more common in advanced age, and generally asymptomatic, geriatric patients should be more careful in terms of RMA, and biomicroscopic posterior segment examination should not be skipped. 3 |
